Dean isn't exactly the patient type; I can see him going a bit crazy in that situation--either on his own behalf or because he wants to look after Sammy and help him out.
With Sherlock... I can see Sherlock trying to distract himself by analyzing the situation at hand, though unable to carry out any plan with a full bladder... or I could see John trying to hide it from Sherlock until the last possible second, even though Sherlock knew to begin with, of course.
Or wherever the muses take you. I approve of both fandoms/pairings suggested!
